New Patients

UPDATE: As of May 2025, Dr Irene Papaconstadopoulos is accepting new patients on a rolling basis, to ensure the highest quality of care for our existing families. If you are a new patient wishing to transfer your care to our practice, please join our waitlist by contacting our office as outlined below.

Newborns should still reach out to us on the day of birth to establish care promptly. Please note that we may not accept some insurance plans for newborns—our team will be happy to discuss your options when you contact us.

Once you’ve joined our waitlist, we can begin the process of requesting prior medical records. Your first appointment for establishing care will be scheduled as soon as an opening becomes available.

We are thrilled and honored to have your children enroll in CHAMPS Pediatrics. For your convenience, we will attach all the required documents that should be filled out prior to booking your first appointment through our secure app "Klara".  It takes 10 minutes only. In order to provide the most accurate and HIPAA-compliant communication regarding your children we use a secure texting app.  We require the following steps prior to scheduling your first appointment:

1. Text us through the "Sign in to Klara" app  (below) or text (808) 698-5565 or call our office at (808) 495-9616 during business hours. These numbers are for non-urgent issues. Request a new patient packet. 

2. Read carefully and fill out one "new patient packet" for each child. In order to protect patient sensitive information, we do not accept these documents through email.

3. We will send the request for medical records immediately, therefore please provide accurate contact information for the previous PCP. It is patient responsibility to ensure we have all records prior to well child visits. Signing the registration form does not confirm establishing care with our practice. Care is established after the first appointment. 

4.  We will send you available days and time slots for your first appointment. If there are specific days you prefer, please let us know.

You can sign up for a prenatal class and/or request a meeting with Dr. Irene prior to your delivery to see if we are a good fit! 

Our next prenatal class is June 7, 2025 at 9 am in our Kahului office.

Once you go to the hospital to deliver, give our practice name or Dr. Irene Papaconstadopoulos, as the Pediatrician and complete the above process to enroll. Call the office at 808 495 9616 to schedule the first appointment which is usually 1-2 days after the discharge from the hospital.

Insurance and Billing:

As a courtesy, we will submit your bills to the following insurances that we are credentialed with. Patients must check with their insurance carrier prior to the appointment to confirm what services their plan will cover when seen by CHAMPS Pediatrics. 

• HMSA all plans, including QUEST

• Blue Cross Blue Shield (please check with your specific plan)

• UHA

• HMAA 

• Aetna (please check with your specific plan)

• Cigna (please check with your specific plan)

• Tricare-West (out of network provider)

(If we are not in network with your insurance, ask us about our payment plans)

** Out of state insurance restrictions may apply for visitors. 

** It is patient responsibility to confirm with their insurance prior to appointments. 

Notice to all patients:

Under the law, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services. 

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service, or at any time during treatment.  

If you receive a single bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Payment Options:

We accept checks, cash or credit cards. We also offer online payments and self pay discounted fee schedules (payment at the time of service).  We will file claims to insurance on your behalf. Our priority is for your children to be seen and do not want insurance to be a hurdle for you.
